About Duke HomeCare & Hospice
Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.

Duke HomeCare, Hospice, and Infusion Services (DHCH) Referral Center
Shifts: Every other weekend and as needed. Orientation will partially be done during the week.
Location: Hybrid
JOB SUMMARY
Coordinate internal and external referrals for DHCH as well as hold the responsibility for initial intake of provider orders. In this role there is no direct patient care.
MAJOR JOB RESPONSIBILITIES
- Provides nursing care for patients in accordance with physician’s order and established policies and procedures
- Participates in weekly team meetings and case conferences, as necessary, to facilitate that appropriate services are provided to the patient
- Maintains up-to-date knowledge of Medicare/Medicaid/JC through in-service and workshop attendance
- Coordinates care plan activities with multidisciplinary team members as directed by Nursing Case Manager
- Documents care and services provided in accordance with DHCH policies, procedures and regulatory standards
- Provides documentation that is timely, concise and complete
- Follows standard precautions and uses Personal Protective Equipment (PPE) as required
- Observes patients’ reactions to medications and reports any adverse reactions to the supervising nurse or physician
- Communicates regularly with team members, caregivers and physicians
- Implements agency policy, procedures and objectives
- Maintains confidentiality of employee and patient information
- Exhibits an attitude which promotes harmony and goodwill in the workplace
- Displays a clean, neat and professional appearance
- Participates in Performance Improvement Program
- Promotes quality, comprehensive services through a team approach.
- Performs other duties as assigned
Education
Work requires graduation from an accredited Practical Nurse program.
Experience
- One to two years of experience preferred
- Working knowledge of Medicare and Medicaid regulations.
- Strong Computer skills preferred
Degrees, Licensure, and/or Certification
- Current State of North Carolina LPN licensure is required or compact license in participating state.
- AHA BLS (May be obtained during orientation)
Knowledge, Skills, and Abilities
- Knowledge of scope of licensed practical nurse, ability to delegate to nursing support staff.
- Considerable knowledge of the care and treatment of patients and special procedures that apply to practical nursing. Able to independently seek out resources and work collaboratively.
- Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others.
- Able to teach patients and families in accordance with the nursing plan of care.
- Able to use sensory and cognitive functions to process and prioritize information, treatment, and follow-up.
- Competent in BLS and/or other specialized life support requirements designated by work area or organization.
- Able to use fine motor skills. Able to record activities, document interventions.
- Able to use computer and learn new software programs.
- Able to navigate the entity to provide clinical care for patients.
- Able to withstand prolonged standing and walking.
- Able to remain focused and organized.
- Working knowledge of sterile techniques and special procedures that are applicable to work performed.
- Working knowledge of procedures and techniques involved in administering routine and special treatments to patients.
- Working knowledge of sanitation, personal hygiene and basic health and safety precautions applicable to work in a hospital.
- Working knowledge of infection control procedures and safety precautions Ability to understand and follow oral and written instructions.
- Working knowledge and completion of appropriate JCAHO and other regulatory requirements
