Department: Community Support Team
Monday-Friday, 8:00 AM-4:30 PM
Predominantly working in the field, transporting clients
The Case Therapist III SL reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Community Support Team [CST] Lead
-Serve as liaison for the CST team across the Community Mental Health Center [CMHC] and in the community through other referring social service agencies, courts, and hospitals.
-Conduct regular quality assurance review of CST records with the assistance of a LPHA.
-Conduct Integrated Assessment and Treatment Plan [IATP] & Level of Care Utilization System [LOCUS] assessment in accordance with Rule 132/140 standards.
-Complete clinical chart review for assigned CST caseload[s] or client referral[s].
-Independent management of schedule [appointments, phone calls, paperwork time, trainings, meetings, tracking travel etc.].
-Document in patient medical record in an accurate and timely fashion.
-Conduct community-based sessions that support client’s progress towards identified treatment goals to productivity standards.
-Conduct client centered consultation with both external and internal providers.
-Provide crisis intervention and safety planning including linkage to other teams/facilities/etc. as needed.
-Conduct team meetings/huddles/supervisions as assigned.
-Provide linkage and referral to internal and external interventions/resources/services as applicable.
-Track client data on a monthly basis to maintain program fidelity.
-Obtain preauthorization/reauthorization from insurance as needed.
-Maintain Leader on Duty shifts as assigned each month.
-All other duties as assigned.
Qualifications
Required:
- 5 years of experience and training in assessment, individual, family, and group counseling
- Master's degree in social work, psychology, counseling, or related field
- Master's degree with LSW, LCSW, LPC, or LCPC, or eligible for licensure
Preferred:
- 5 years in intensive treatment setting and/or mental health setting
- CADC
- LIC CPC
- CERT CADC
- LIC LCSW
- LIC MSW
